Medicare Part D Creditable Coverage Disclosure Requirements
The Medicare Modernization Act (MMA) mandates that employer group health plan sponsors disclose to Medicare eligible individuals with Rx coverage under the plan whether coverage is “creditable”. Employers must make this disclosure to Medicare Part D eligible individuals no later than October 15.
The employer, not the insurance company, is responsible to provide notice to the Medicare Part D eligible individuals.

A separate electronic disclosure of Creditable Coverage status must be made by the employer to CMS no later than 60 days after the beginning of each plan year. For more information go to cms.gov/CreditableCoverage/45_ccDisclosureform.asp
